Step 1. Please review the following enrollment qualifications.
Are you eligible to enroll in a Medicare Advantage health plan?
Enrollment periods vary if you did not recently become eligible for Medicare. You must be enrolled in both Part A and Part B and can apply for coverage three months before or three months after your Part B coverage starts. If you are not enrolled in Medicare Parts A and B, you must first enroll online at www.medicare.gov.
You must permanently reside in the service area covered by this plan.
You must not have ESRD (End-stage Renal Disease).
Additional requirements apply for SNP (Special Needs Plans) enrollees.
You may not be enrolled in more than one Medicare Advantage health plan at any given time.
We will inform you of your effective coverage date, based on the information you provide on the enrollment form and your Medicare eligibility dates.
You can enroll in a 2009 plan if you meet specific qualifications. These include:
You recently moved to a different area
You are turning 65 this year
You are over 65 and retiring (or losing coverage through an employer plan)
You are no longer eligible for Medicaid coverage
Your former health plan insurer has not continued medical service in your area
